Incidents

Incidents are be created in the related Procedure where the Incident occurred. They will go through the following steps.

  1. An Incident is created: Safety Officer only

  2. Identify details of the Incident: Safety Officer only

  3. Identify users involved in the Incident: Safety Officer only

  4. Identify user injuries: Safety Officer only

  5. Optionally, add an Investigator to include their follow up: Safety Officer only

  6. Make a recommendation of how this might be avoided in the future
