Incidents are be created in the related Procedure where the Incident occurred. They will go through the following steps.
An Incident is created: Safety Officer only
Identify details of the Incident: Safety Officer only
Identify users involved in the Incident: Safety Officer only
Identify user injuries: Safety Officer only
Optionally, add an Investigator to include their follow up: Safety Officer only
Make a recommendation of how this might be avoided in the future
